Part 2, "Once", the young man goes insane and goes on a killing spree. Part 3, "Footsteps", is the young man looking back on it all while in a prison cell waiting for his execution.

it's a personal account. eddie thought his stepfather was his biological father until he was a teenager, when his mum told him his bio father was someone else who had died of MS when eddie was 13. he had met him as a child, believing the man was an old family friend... eddie has said he always felt like this was a 'curse', but that when audiences started singing the chorus as an affirmation of life, the song transformed his feelings about the situation and the 'curse' lifted.
