Kurt used to make and decorate lots of little heart shaped boxes for Courtney Love as presents. This song was written once Kurt started getting bitter about the amount of control Courtney tried to have over him and his career.

Some say it deals with Courtney and how she would give him heart shaped boxes, I've read Kurt was inspired to make this song when he saw something on T.V. about children with cancer, hence the line,"I wish I could eat your cancer when you turn black,", it could just be dealing with love in general, or even him showing the way the media though of him, hence the line,"Hey! Wait! I've got a new complaint." It's up to you really.
