It's the same as the triple jump on the Prologue. Hit jump at the exact instant when you land on the pipe. Search for pipe jump on google and you'll find how to do it.

I am not sure I understand exactly what´s going on. Does your game go back to the main menu when you finish a chapter or simply during a chapter? Have you already completed the game once? - If this is the case the behaviour of the game is OK - it lets you play only one chapter and it does not continue to the next one. But if it´s not the case I do not know what´s wrong.

I have figured out the mechanic of the first jump starting in 1:51. To those starting to practice this jump start out by using slow to get a feel for it. Here is how it goes:
Approach the seat head on, press slow mo. Once you are in slow mo, hit the jump button to begin the jump animation. then in the middle of the animation with the RIGHT ANALOG turn 90 degrees, you'll be on top of the lamp. Once there wall run up turn 180, then RIGHT analog right to grab the handle.

so at 1:51 he vaults off a chair, turns immediately to his right, uses the momentum from the vault to do a wall climb up the pillar, turns, jumps off the pillar, turns a little to his right, then climbs over the rail.
at 1:57 he vaults off the box, turns to his left, wall climbs, turns, jumps, and climbs up rail. Basically the same as at 1:51

From what I have gathered from the video, I believe its: Running with momentum towards the chair, hit jump button, in the middle of the vault hit the 90/180 button, then hit jump, then wallclimb until the top, then hit jump, then turn controller to the left.
I haven't been able to turn Faith 90 degrees once she begins the vault jumping animation. Can you describe it in even more detail Metalmurphy?
Make sure u run to the step already looking a bit to the right, it makes things easier, as soon as the climb/jump animation starts keep turning right and pressing L1 (jump), let her climb a bit and then hit R1 (turn) and L1 again and just keep turning right and pressing L1, u don't have to be precise here, just keep pressing L1 while ur turning and she'll grab. If you keep turning right and pressing L1 the camera locks to the right place when its time to grab. Thats why its easier then it looks
