Firefighting uses complicated and technical tactics that are hard to understand in the general public.

While transformer oil is a carcinogen and hazardous in fire situations, the tactic being used is to plug the hole. The hose is attached to a special nozzle. It's held in place to do it's work. The water turns to steam which will put the fire out. But has to be enclosed for the steam to work properly. This type of fire can't be fought directly by normal means.

Most likely the gases/fumes from the sewer came in contact with exposed wiring, thus igniting the gas. It happens alot, but it usually doesn't sent manholes flying in the air, let alone a huge pile of smoke.

Could be a similar problem to New York. The insulation on old wires starts to rot and turn into a flammable/explosive gas. As it wears away, the wires spark and ignite the whole mess.
