It doesnt hurt, you need to relax your arm because they shoot it into your muscle. Therefor if you strain, your flexing, and that makes the needle harder to get through the muscle, so if you leave your arm completely relaxed, you dont feel a thing. The chickenpox shot is different, the needle through the skin hurts, but not once it gets through the skin.

The reason tetanus shots can hurt like hell or not at all really shows how good the nurse is at vaccinations. I got a tetanus shot the other day and for some odd reason I just couldn't stop laughing...
