In addition to what other people have said, I would suggest just put the incline on the bench a little bit lower to take the pressure off of your shoulders. You'll still hit the upper chest area, but the movement won't be awkward for your shoulders.
That little change helped resolved my shoulder and elbow problem.
Actually, there are a few different ways to perform chest exercises which all depend on the area you are trying to target. Erin's form is great but the weight might have been a little excessive for that specific form. Shawn's explanation is great, however that mainly applies to powerlifting (heavy weight). Bodybuilding is different from powerlifting...if you are going for max weight, use Shawn's technique, if you are going for definitiion, lighten the load and keep your elbows out like Erin.

@erinstern5 See how you are over pronating your wrists almost like you are trying to get your elbows higher. This position puts your glenohumeral joint in a very bad position and can injure the supraspinatus muscle in the rotator. From my experience (now I am a powerlifter) turning your wrists to a 45 degree angle and tucking your elbows and pinching the middle back helps take stress of the shoulder and activates the chest more, and it also helps increase power output

Nice slight arch in lower back. I do the same to were my tailbone and shoulders are the only thing touching the bench. That way it really concentrates on the upper chest.
