The Witch is in the SkyBox which in L4D1 is located beneth the map. its a tiny wee city. The 3D skybox scales the contents of its "area" and makes it 16x bigger using an entity called Sky_Camera for the field of view. Skybox's are usually hidden somewhere out the map, especially in L4D2.

Make your owner server, activate cheats, then noclip downwards until you see a small area with clouds around it. That is the skybox.

The reason zombies appear so big is because the skybox in this game is so small. In games like Garrysmod and a few TF2 maps, the 3D skyboxes are generally bigger.
