..pretty sure they made it from a melodica..a small piano looking instrument with a mouthpiece at the end..(made by Hohner originally). You might be surprised by the vast # of types of harmonicas out there: bass, chord, diatonic, chromatic, tremolo, etc. ALL kinds are played at harmonica conventions/festivals. Trios usually consist of chromatic, chord and bass. Jason Ricci, a brilliant modern diatonic player is now studying/playing chord as well. On UTube: watch his 'harpin help' first. (cont'd)

no, the best harmonica players don't play guitar and harmonica. I'm assuming you mean a harp in a rack...guitar at the same time? 'Some' do, some don't. A few of the best living harmonica players? Howard Levy, Jason Ricci, Kim Wilson, Robert Bonfiglio (Chromatic), Rob Paparozzi ...Charlie Musselwhite, Charlie McCoy...there are tons more. I could list dozens. Of those, the only one who plays guitar is McCoy as far as I know.
