My 3 year old daughter has juvenile arthritis, and is with Cigna. There's NO WAY I can fire Cigna no matter how lousy they are and no matter how much more they raise rates, because nobody else will insure her due to the preexisting condition.

A similar factor is at play with laser eye surgery with more and more doctors performing the surgery. Please note that in each case the manufacturer or doctor profits from their creative work. In 1973, the Federal Government passed the HMO Act that forced large employers to offer HMOs to employees.
