I think the game speed is really fast. He takes a look at the positioning of the coming up blocks and thinks about how many time he has to press the rotate button. Plus his aiming (left right) skills are probably automatically and as you see he has somewhat the same strategy all the time. Still very hard tho. This are some 1337 skills :)
