How do you monitor wiring that looks perfectly normal? the IFE system hadn't caused any problems up until the time of the accident and it was stated clearly in the programme, that the plane wouldn't have made it back to Halifax under any circumstances. I suspect the crew did as well as they could under the circumstances and can not be held accountable for the crash.
The fire was actually far more severe than this documantry depicted. Fire actually was raging from the overhead panel, Checklists were found molted together, which indicated that one of the pilots and likely Captain Zimmermann was trying to fan off the flames.
Whilst sympathising with the father who lost his daughter, it wouldn't appear that Swissair per se, were responsible for the accident. It was due to a fault in the aircraft that could have happened to any carrier.
@chriskell9 This accident is the responsibility of the IFE company that installed the IFE system, Swissair, and the crew. The IFE system was installed improperly and the wiring arced and caught the insulation on fire. Swissair maintenance did not monitor the condition of the wiring and did not ensure that the system could shut down if the temperature got too hot. And last the crew was responsible because the Captain wanted to dump fuel and wanted to fly by the book and didn't land immediately.
There is one thing that shoudl be mentioned in this doc that wasn't, and that is these pilots are really heros, because while I think most people, inculding myself, would have skiped the books, and just tried to land the plane, it seems from the moment of the pan pan pan, this plane was doomed to crash, and had it been over land of halifax, would have been easier to investigate but it might have taken more lives than it did, so god bless the polits for saving as many lives as possible.
@KangaKucha my uncle works as an plane mechanic and he always says MD-11 is a plane that should have never been made. It has so much flaws, always technical problems and it has been involved in so many accidents. He himself has said to my family to avoid this plane type. Now it's easy cos almost all of the airliners have given up on MD-11. I agree it's a nice looking plane, but the design is big failure and so many people have lost their lives or injured cos of it :(
@missfinlandia88 do you mean the DC-10 or the MD-11? because the DC-10 had way more incidents than the MD-11, and ensured that the 11 would never be sucessful.

My dad worked all of his life in aviation and ended up getting fired in the PATCO strike of 81. He passed on his love of aviation to all of his kids, which is why I'm watching this on my Saturday.

i feel sad for passengers.
if i was a pilot, smoke on the cockpit means LAND ASAP at the nearest airport. no leveling, no by the books, no fuel dump!
