He does it to prove to Martha he's a time traveler, he goes back in time to this point, before she enters the hospital, and takes off his tie in front of her.

This, in turn, causes Martha to question why he didn't just tell her not to go to work. He responds: "Crossing into established events is strictly forbidden. Except for cheap tricks."

Easy. This is the beginning of the episode. At the end to prove to Martha that the TARDIS travels in time, he travels back to that point, shows her the tie, that it was taken off. Then he travels back to when he was talking to Martha.

As the 5th doctor (Peter Davison) would say:

Because at the end of the episode, Martha Goes, No Way that he can travel through time. He does so and goes back to that point and goes: Like So. See? And then goes back to the TARDIS and then when he gets back to Martha, he's putting his tie back on. Get it?
