actualy, i have to dissagree with you. i used to have that problem. heres the evolution of my answer:
wen playing on my laptop, arrows cannot be used, so i used w,a,s,d with my left hand, and then i,j,k,l with my right hand. took a while to get used to, but after about an hour i was pro.
for my desktop, i use w,a,s,d left hand, 4,5,6,8 on the number keypad with my right hand. anything done with the numberpad does not interfere with keyboard, so i can push 2 numbers, and 2 buttons from KB.

The four directions arrows At 1:04 is too hard.
Because some of the PC keyboard can't push 4 keyboard at the same time.

My keyboard doesn't allow my old setup for spread which was 'askl'
I have since moved to qw[] and qw89 for left/down/up/right.
Find something your keyboard can do - Stepmania has a "test input" thing in the options there.
