Question: What is the meaning of the phrase from the Prayer (English V) "whether in this life or whether in a previous life"; I can only assume that you may (hopefully) be referring to harm done to our ancestors that can translate to us? We know that Torah does not teach Reincarnation...

Reincarnation is a given in kabbalistic literature from the oldest kabbalistic texts (i.e. the Bahir and the Zohar) to the writings of Rav Yitzhak Luria - the Arizal. The Arizal's writings contain an entire book devoted to the topic of reincarnation. Reincarnation was accepted by more recent rabbinical leaders such as the Vilna Gaon, as well.
