It's not supposed to be consecutive, it's cumulative.

You have to do 100 (or 200) Pull ups altogether.

It doesn't need to be all at once, you can do 3 pullups, raise yourself up (Triangle if I remember correctly), jump over, do three more, and rinse and repeat until the Bar turns a different color.

For bandanna you need to collect 60 dogtags in different difficulties, 100 for stealth

you need 120 in Plant for Stealth Camo

If you collect all the ID's (Better known as Dog Tags) you get the infinite ammo bandana for use in the next playthrough on that save file. :3

Most Russian soldiers understand and speak English, some of the Russian military isn't even Russian so it's possible for them to use it as a default language.

@REMBERvad Actually, snake knows 6 languages, and considering his enemies one of them might be Russian. It could be that they're speaking Russian, snake understands and translates. Remember, snake is telling the story. Y'know when he said "Two years ago..."
