Does the towel really retain any heat for a good amount of time? I find that you have *maybe* a minute, at most, before you lose the heat. Especially when moving the towel so much.
is the patient very tiny? or is the therapist a giant?

Ya, it's about a minute or so. But if you place a dry towel over it, then you can retain the heat longer.
