this song was called 'subtle' before. It didnt occur to me until now that subtle is such an inappropriate for this song, so wonderful and all the details are beautiful.
Oh and at 3:06 when Thom starts moaning and Jonny just does his little idiosyncratic messing-with-the-hair before going back to his guitar. His modesty... it eludes me-- it kills me. It's like he's saying "Yeah, I play multiple instruments rather decently, I suppose. But I couldn't live without Radiohead as a whole."
Also, there's some serious Dorian Grey shit going on in that celestial being.

It was certainly not a mistake...whether it was planned or not you'd have to ask the band. Either way it is an inspired moment, underlinling the "lonely" text playing his guitar and singing...alone. It works...

i would reckon that the ending was planned, because of the lighting effects...a spotlight was put on thom a bit before he even started his solo segment at the end
It seemed like a deliberate tribute to the other members, to children, or a personal something. Also I think it was a deliberate exposure of those lyrics in a sensitive quiet way and to add interest to the set.

It's meant to be music. if your listening/watching for precision, check ping pong out it's amazing. Or Take it for what it is and don't worry wether it's a mistake with a classy reaction or whatever, that's why i love Radiohead, because there unpredictable live and always pull it off to the highest degree possible.

Spontaneous or planned, in mho, it is the sign of a great band that these little variations and reprises can be thrown in in a live performance. It only adds to the artistry. Some of the best footage I've seen of this anthem.

I think at 3:46 instead of going to the rock out part Thom start another pass. Then at 3:58 Thom gives the universal singer signal that the change is coming by waving the guitar headstock up and down.
Ed's unsure look at 3:44 was hilarious indeed. Also that and maybe a couple other things give away that Thom had forgotten the lyrics or skipped them or whatever and that it was not "planned." Maybe Thom had done that at a previous performance like someone pointed out but he probably also sang "We are accidents waiting to happen" 4 times like he did here. Thom's ending was great, though. :D
