The pen obviously malfunctions. Count the number of times Boris pushes the lever. Remember, 3 times to arm with a 4 second fuse, and another 3 clicks to disarm. Several times, it takes Boris far longer than the 4 seconds to click it 3 times to disarm it, meaning it should have gone off several times.

It's incredible how Serra's score gives the movie such a cool feeling. A feeling that you are actually watching a spy film that is full of suspense and mystery. I think they have failed to recreate that vibe ever since, musically and visually. Definitely a sign of the times too. Way to go Bond producers!

@DonLduk I'm not sure, but I THINK it's because Boris didn't click the pen 3 times in succession. He always either clicked it twice and paused before clicking again, or he clicked it 4 times. It wasn't until the end when he clicked it 3 times and then ran up to Natalya and Bond.

The reason it's so confusing to a lot of people (especially when I was a kid) was that it has to be 3 in direct succession, like you said, but you never actually SEE him click it three times. After he drops the pen, he picks it up, and yells at Natalya. If you listen, as he stands up to yell at her, you'll hear the pen click three times in rapid succession, and I'm pretty sure Bond's reacting to that more than Boris getting up.
