Partial answer to my own question: A while back I had a chance to attend a con Q&A session with Mike McFarland, an ADR director for Funimation, in which someone asked about the inclusion of outtakes (or lack thereof) in their titles. According to him, Japanese producers often don't "get" the concept of outtakes - they don't understand why the recording studio would want to show audiences the actors' mistakes, which they believe ought to be concealed.
