you can do this on any mode, even if the hidden hats aren't visible, if you memorized the locations and shoot it, you're good to go

You can still do it on other difficulties but the hats are invisible so you have to memorize where they are.

No, it only happens when do the Hat Easter Egg and die when you're supposed to shoot the driver of the other RIG when you try to cross the bridge.

I did a little experimenting with this, and you do not necessarily need to be on the Insane difficulty in order to use this easter egg, juts to see the cowboy hats. This can be done on all difficulties as long as you know by heart the locations of the hats.

True, but this seems to only give hats to Marcus and the anonymous Gears, but not Dom and Carmine. I don't know about the corpser. The only way to get hats on everyone, though, is by doing it on Insane.
