ok the 2 head is a puppet then he puts it in the box also conetting strings then it flys up he stuffs his head inside his shirt puts the puppet on his neck then brings his head out

well its pretty simple, when he starts to fight with his head at 5:30, acts like he is covering him up at 5:45 that is when he takes his hand out of the second head and puts it in his right arm which must have some hard material, and thus making it look like there was another head, the rest muct have been strings -.-
