@iejelliott Nobody can render tf2 like that. If you save a replay, it creates a render. Depending on how smooth you want it, it can render anywhere from a few minutes to hours.
@ProjectNeuroLogic - This is actually a kind of neat view into the technical aspect of demos, and why they hate rewinding so much. It's basically the fact that a demo does not consist of every frame of a match, but just instructions on how to build the match in the engine. Think of some event that starts the medibeam particle effect, since the demo starts after he had started healing the heavy, it remained invisible until he switched patients and the event was fired.

I may be wrong, but i think tf2's strategy is a fairly straight forward one. To fast-forward, play the game super fast to the point. But while it plays a demo it makes temporary snapshots of the gamestate every x frames, and to rewind it either fast forwards from the beginning to the point, or from a snapshot to the point. Whichever makes more sense.

Apparently during demos, the beam graphic only functions when you stop&heal again, as I've yet to see a beam from the beginning of any videos. Interesting indeed, my friend :D
I have tons of hours as Medic and I never once thought of using ubers to stop backstabs.
Then again, I main kritz, but still! I applaud your creativity.

@iejelliott When creating demos, you can enhance the graphics and make it alot prettier. From what I've read (and been told), this is the case.
