I am pretty sure the super high falsetto on the last chorus is Jimmy. Listen to the backing vocals to Almost easy. They sound identical. Jimmy does a really high falsetto in Almost easy as well.

It definitely is still Matt singing the higher parts in the chorus. You can really hear it when he says "GREAT" at the end of the chorus, he makes the note fall down and it's obviously his voice. Also, he usually does "scream a little" when he sings; however, that didn't even start happening until this album. His clean vocals on "Waking The Fallen" and even on songs like "Warmness on the Soul" from Sounding The Seventh Trumpet were not gravelly at all.
