If you look at 0:50, it appears the track marshals were all putting out their yellow flags -- it looks to me like the Safety Car was being deployed (even though the graphic on the top of the screen doesn't show it) and Viso just didn't see everyone slowing down.

I agree. Of course at the same circuit in 1995, just coming to the hairpin preceding the site of that accident, exactly that eventuality (head hitting the top of the wall) killed Marco Campos.
