I assume this patient was unstable because you're attempting to cardiovert his wide-complex tachycardia without IV access or sedation but you did not verbalize any other clues that would make clear the patient's hemodynamic status. AHA guidelines are 100j for cardioversion of wide-complex tachycardias.
i understand that this is just a demonstration video but i have a quick question. I was under the impression that for a wide complex tachycardia with a pulse start cardioverting at 100 J ? is that just my region? i know for A-flutter and SVT you start at 50 J. or am i just confusing myself?
You could be right. In our region, we have to call a signal, to the MD, before we are allowed to continue with synchronized cardioversion. The MD could follow AHA guidelines or they may choose differently. In that case, the joules could start at 50J, 100J or beyond. In my region, it all depends on the MD.
