Well, I think it's b/c he lives in such an impoverished area/ developing country.

As you can tell, his village is remote, area is impoverished. These areas have difficulty getting medical attention for little things, let alone such a big tumor. Money is another issue- folks in these places usually live on as little as $2/day. Surgeries are kept for the upper class and cost much more that they can ever afford.
