i have never played this game i think the story line is clear. he was hit by a car and put in a come, when he wakes up there appears to be a zombie apocalypse, just 'the walking dead'
for me, the second ending worked, and it kicks ass. the technical problems like you going blind IS your drivers and when you think you got hit, look at the life meter, and if you go to 1c's forum, they explain why the game is as strange as it is

@TheShenmoki Really? The second ending worked? I remember seeing an interview with the creator of the game saying he ran out of time and couldn't work the second ending in the game.

Having played the game, I can say that I liked it's campy/cheesy factor.
