It was an accident during a botch. C-4 swung a bit stiff as Hazel was attempting to duck away from the move (Out of fear, some people are afraid to get stiffed) and ducked the wrong way right into the oncoming last shot that broke his jaw. It goes to show that you should have full trust for your opponent and if you have a fear of taking a move or perhaps a shot from a worker who may be heavy handed, wrestling isn't for you.
