What makes a swatch different from a Jersey or a Patch?

Swatch and Jersey are the same, just depends on the name on the product. A patch will have part of one of the logos or nameplates or something, not just a plain piece of jersey.

the actual fabric is the jersey/swatch and the horse guy is the patch

No, Tom, actually this was a prize in a New Jersey scratch-off lottery ticket in 2004, signed by all three and numbered 133/334. I found it on ebay a few years ago and had to grab it!
