It actually does happen fairly often. Clearance awareness is a difficult concept for a operator that is always used to clearing things. After working 14 hours, I had an accident a year ago loading a trailer. As I entered the trailer, the impact of my lift sent a faulty trailer door rolling down. My mast caught the bottum of the door and bent it. I was so exhausted that I didnt even care what might become of my employment. Everyone understood and the carrier didnt charge us.

sometimes it's not the forklift operator fault
it's down to the one that open up the warehouse I.E The team leader or manager
never leave the shatter half open or this will happen.
happen in my warehouse about 6 times.... sadly always the team leaders, they blmae the workers for now opening the shatters.
