As a makeup artist what I would do for the sylar effect would use blood and gore makeup and put it in a line on the forehead. Next you would want to place a long strand of fishing line straight on the gory line. Lastly you would want to put flesh colored liquid latex on the whole thing, try to make it blend as well as you can to make it look normal and look like the front of the forehead. When you take the shot just pull the line, it will pull through the front layer of latex and show the gore.
