Looking through a 1943 Kai ROM in MAME i found something that could either be a Beta Leftover or evidence for a scrapped U.S. release - Among the title graphics is a giant "MARK II" that wouldn't be out of place underneath the 1943 logo.
Either Capcom called it "1943 Mark II" during development, or the planned to release it in the US as that name. Huh.
Now I know why the continue option is not available in the Japanese version. You get a super bonus of 1,000,000 points for completing the stage, which is not on the PS2 version (USA)
I remember credit feeding the Japanese version thinking oh yeah I can beat this warship. Long story short, I died, and the game doesn't let you continue in the last stage, unlike the US one. I was mortified and vowed to one day beat the harder Japanese version (the last stage seems to be the only difference continue wise.

Hmm...what game are you talking about? The Japanese release of 1943: The Battle of Midway? Or 1943 Kai (this game), which is Japan only?

@Seahawk0027 1943 Kai, this one, included on Capcom Classics Collection 1 on PS 2. I was surprised to see it on that collection.

Yeah I suppose credit feeding to the end and then being denied on the final stage would be pretty lame =P.
