@VickiRDH thanks for the reply... make sure you are not missing the file 'register.php' and that it is stored in the correct location and is accessible.

the error 'failed to open stream' means the file couldn't be located by php.

try putting all the code in one file and see if it works then one by one put the respective code in separete file where you want them to be.

@warfare73 Well if you are using php technology you need to first have WAMP on you computer. It stands for Windows Apache MySql PHP and this WAMP will install all this in one shot.

You need just 3 things to setup:-

1 - Define proper column name

2 - Define proper Table name

3 - Define proper Database name
