They are planned with the winner decided. Sometimes it gets changed right before the match and sometimes the finish is worked out. In some cases, the wrestlers practice and plan the whole match (Hart vs Bulldog Summerslam 1992 and Savage vs Flair Wrestlemania 8), whereas in other cases, they just wing it until the finish.
