@Superlube100 The point of the whole movie, is that even if you are surrounded by iron bars and stark white walls, you can always be freed by the inner universe of your mind. You can make what you want of any situation as long as you can change how you view something, then you can change how it makes you feel. The surgery thing was quite literal, but it also was the physical representation of what she had already done by escaping the harsh reality of things by kind of sugar coating.
