REMOTE puts outside insulation on the entire house. (roof and foundation) Just putting above grade wall exteriour insulation will not do the trick. Insulate all the framing or do nothing.
What are you actually mounting the windows to, since it looks like you only have that thin plywood window buck/extention there? I would think that this would not be strong enough, and instead of pw, you should maybe use a 2x material???
Avg extra cost.....@4000$ Cost per year with 7% morgage @300$ Savings per year with a 16 SEER 90 AFUE unit @ 100$/year. Oh this is why that isn't the standard in new construction? So many builders ready to spend a dollar to save a dime, so many customers willing to jump on the bandwagon, till they get to choose granite countertops, or foam board, fck the foam board. Granite may actually have a return on investment, by increasing the value of the home. Nobody will ever know/care about the foam
This is awesome, and looks very good. Since you build the window boxes to accommodate the width of the insulation, it doesn't look like this could be done on a remodel.

Great general guide here, thank you for posting. One question though:
At the 90 degree flashing face where it meets the rigid stuff, do you caulk that? I'd imagine there is air loss if not.
