I think that the 40% who earn over £34,600 pay about 40% to the government in tax. Which in my opinion far too steep. That bracket purchase more taxed goods, which makes up for the difference anyway.

But the people who can afford private care are the ones who use private care at the moment. The only way in which your plan would lower spending is if the threshold was set artificially low so many middle-class are forced to pay for private treatment; worsening their position in favour of the rich. Effectively the middle class would end up paying twice.
