I learned this by ear so I don't have the sheet music. I don't have the time to write the sheet music but I have all of the left hand chords in the description to help you out. The chords are just the first note, fifth, and upper root. For example, for D major I play D A D together. The right hand doesn't jump around that much if you watch the video. Only 8 keys are used. Hope that helps.
