Could be he suffered from an arrythmia. According to one study a staggering 70% of freedivers experience premature complexes, predominately at the end of apnea. It could be the reason he blacked out earlier than usual for him.

At min 1:27 you can see him wearing a t-shirt with the logo of Ebansub,an excellent italian company of handcrafted wood spearguns,owned by a guy named Giulio Campopiano,who was an experienced freediver and spearfisher too and died in the same misterious way during a freedive some months ago at Isola d'Elba.His body was never found,they could only find the speargun with a fish on the shaft.
