I'm trying to figure this out myself, judging by the video and hearing it on my acoustic I think it's:

End of first chorus: Em D C Em

End of second chorus: B5 A5 G5 A5 ... in case you don't know, power chords rooted on the low-E string at frets 7, 5, 3 then 5 again for one last strum.

Anyone who tries this out please comment back, I'd like to know how I did.

After trying it once more I think I had the second chorus wrong too.

E5-D5-C5-D5 (aka power chords rooted on the A string, frets 7-5-3-5) sounds better than what I said in my first comment.
