Odd. On the PC I got no second chance. It would go off and you had to get away to let it reset. Made this mission quite hard and frustrating after a while till I got the hang of what letters/numbers it used for the sequence.

Hm, your ps3 controller seems to malfunction too often. Perhaps you should check out if something is interfering with it (wifi, bluetooth, other wireless electronics) and turn them off or move them away while you play
