Several mistakes: Depending on yeast quality, it is always not advised to melt the yeast with the salt. Yeast should be disolved in another container with warm water around 110°F.
The dough ball should be smooth, and for that you need to knead it for around 10 minutes before letting it rise.
Also you do not let it rise enough, with your receipe I guess 2 hours at room temperature is a minimum. I advise letting it rise slowly for at least 2 days in the fridge in a hermetic container.

You're adorable and did a great job but you need to double check your ingredients as you didn't use warm water for the yeast and the dough takes longer than 15 minutes to raise. Keep working at it you will be a pro someday.
