The same sort of thing happened to me, but with a Super Mutant...I was mutant hunting in Downtown D.C., when I found a trio of them. I killed one of them in VATS, business as usual. I killed another one while my AP replenished, and when I went back in VATS, I was targeting the last one, on the ground, as if it had been paralyzed! Then, I see Dogmeat attacking an invisible enemy, red crosshair showing. I turn back towards the body, and its gone. I turn back, and see the mutant visible!

i was shooting at it for a while with fat mans,missle launchers and other big weapons...it never lost any health but then i went up closer and it dissapeared but it popped up behind me. :p
that happend to me the very first time i saw a ghoul it it freaked me out and it was on the ground i was just about to kil when it scrached the fuck out of me then it was invisable and that was the first time i saw a ghoul in the game
you can make a darts thrower when you got the plan for it. It's not powerful but it cripples any limb in one shot (pretty usefull against deathclaws they're much easier to kill when they don't run, i always keep one with me^^)
i have the pc version and never had problems like that the only probs i have is finding enough ammo when i do run into the gits (i have marts mutant mod it = swarms of at least 50 of i have 3 heavily armed followers and barely survive an encounter :P)

i've got a glitch in my game too... on the radar there is always a red spot that follows the radar wherever i turn and i even started a new game to try to get it off but the red spot didnt vanish...
For the ghoul mask you have to do a mission for the guard captain at tenpenny tower (usually posted at the main gate leading into the courtyard). Instead of killing the ghoul he puts a hit out on you can either work for the ghoul or try to reach a peaceful agreement (which requires a good speech skill)
I was at that place were u get your dog and like my guy jus kept jumping.....it would let me shoot but like i'd miss with and without VATS....and i'd shot at one raider and missed because idk i was jumping at the time next thing i know i have like 6 of them chasen me....
those things freak the shit out of me lol i jump everytime i have to shoot one, or every time im walking and one comes running at me from the side lol fark i luv this game
There was a ghoul behind him. just not in a body. That similar glitch was in Oblivion, a guard was lying on the ground, but still the player got attacked by the invissible ghost of the guard.
One time my Dogmeat did something similar where he disappeared, but I could still hear him and he was still picked up by my radar. It's like everything but his physical model was loaded.
It was annoying, I had to load to an earlier state or permanently lose him since I couldn't command him to do anything.

If he's talking about the T-51b, you do the quest Crowley gives you, and then let him walk away with the keys.
He'll come back a short time later wearing T-51b Power Armor from Fallout 1.
So you can either steal it from him or kill him for it.
I just planted a grenade on him and took it from his corpse. :D

You have to help Roy Phillips take over Tenpenny Tower, and he'll give you the ghoul mask so during the quest the ghouls won't attack you.

Sorry, i'm not seeing anything wrong.
It looks like two ghouls were chasing you (you killed them) and another came from the area to your left as you killed them.
Ghouls like to hide in places such as that.
Your video notes couldn't have been in a worse place either, you can't see the threat radar half of the time.
