Real, I had chemical burns that ate away at some of my skin, so they did debridement of the areas, to have some of it heal up under dressings before placing the skin graph on top attatched by staples. There are no more superficial veins because the area that feeds the skin is gone, the major veins are still under the muscle attachments. They only remove the skin, fat and whatever is in the top layer of skin. It will still be a scar later after fully healing, and not much feeling in it.

- it isn't gushing blood because in using the right implements, you can (as has been done here) avoid severing or perforating any blood vessels, which would be what causes bleeding.

- as for what he did. Not sure what the reason for this kind of surgery would be, but since he is clearly awake, it is reasonable to assume that it is not an emergency surgery.
