More spikes, items shuffled around so everything is more difficult, terrain changes to make you take routes only available to those who have mastered the wall jumps and bomb hops. 

Notice how the Varia Suit was behind Ridley instead of in Kraid's room. Did you take constant heat damage fighting Ridley in the original, nope. =D

Infernal also makes a really good version of "Self Control". Laura Branigan's original version is classic. De-Tech's version is faster than all other versions.
