@eggfillet they cut that part from the plot. Ethan was originally intended to have a supernatural psychic connection to the origami killer. The idea was scrapped, but certain scenarios had already been written, leaving holes in the plot (why does Ethan wake up with Origami figures in his hand? Why does he black out in the first place?)

I don't think it helps that it looks like the car is crawling at about 5 mph, and that they didn't actually show anything but the mom reacting afterwards. I have a feeling the creators didn't know how he died either, so just didn't show it.
