Could be from damage to the ear. Sometimes when dogs shake their heads hard, they can end up smacking their ear(s) against something solid, and cause enough trauma to the ear (usually vein inside the ear) to 'brake' it, causing a permanent flap like that.
Don't know if that's the case with this dog, just a possibility.
