it's brilliant! reminds me of body dysmorphic disorder, where they exagerate their body image flaw, her hands aren't really that big, it's in her mind, her insecurity, and when she goes to the cage you see the normal size. she has bdd because her mother has been instilling a look of perfection in her, you see that with her and the dolled up look (makup, clothes, hair) but also treating her badly/abusive (like she's a freak when she's not), therfore she thinks its because she's a freak.

Speaking as someone with messed up hands, I just keep them in my pockets to avoid stares in public. I think the character isn't ashamed of her hands, she's ashamed of her face. Hence covering it up. Like other's have mentioned, in the shot in the cage, her hands are normal. We see how her face is covered in make up. The mother is a psycho who likes dolling her daughter up (there's a shot of dolls on a shelf) and keeping her in a cage like a doll on display.
