Okay, that's the point. Wearing PJs to go out is not simply not dressing up, it's actually dressing down. It says you care so little, that you are not willing to go through minimal effort to put on something that is non-sleepwear after you have gotten out of bed. So it implies you are probably to lazy to even brush your teeth or wash your face (even if you actually do). That by definition makes a person seem trashy, when they care that little about personal appearance/hygiene.

Cloths are a statement, if you Do practice good hygiene THEN change into sleepwear, you are basically saying "I take care of myself, but I don't want people to think I take care of myself, by looking like I am so casual that I didn’t have the motivation to change.". Because if you are in PJ’s people assume you never changed out of them after waking up. You may be blissfully unaware that that is how you seem, and maybe your friends know that isn’t true, but no one else does.
