It's possible that they use bacteria, like yeast in bread. The yeast inflates the bread when it's heated up, then it reproduces. The could put the tiny spec of caramel, and while the bars are sheken, the bacteria reproduce and exctrete glucose (sugar/caramel) to fill in and inflate the caramel pockets.

Well, in order to make a mass quantity of bars they would have to build them on an assembly line. Thus the bar would have to be constructed in such a way as to accommodate mass production. So you probably have the bell top of the bar in long waves of molds that are chilled and the chocolate is sprayed across them at timed intervals while the molds pass, the chilled mold is filled with a wave of caramel and then the whole mold is wave washed in chocolate. Later on the bars are cut out.

They make the top part and turn it upside down and fill it with caramel, then they put the bottom part on top of that and turn it the right way.
